Authorization to Administer Medication

Medications may not be sent to school with a child for self-administration, and students may not carry over-the-counter drugs such as pain relievers (Tylenol), inhalers, or cough drops. School personnel must administer all prescriptions and over-the-counter medications. Before any medication may be dispensed, the school office must have a completed Medication Administration Authorization Form (below) on file.

The medication must be in an original bottle that comes directly from the pharmacy. The first full day’s dosage of any new prescription should be administered at home.
